He argued that our instinct to be considerate is even stronger than our instinct to be self-serving.As obvious as that may seem, its only recently that neuroscience has been able to explain why. Research conducted by Dacher Keltner at Berkeley showed that our brains react exactly the saatkorn when we see other people in pain as when we experience pain ourselves. Watching someone else experience pain also activates the structure deep inside the brain thats responsible for nurturing behavior, called the periaqueductal gray. Being considerate of others is certainly a good career move, but its also good for your health. When you show consideration for others, the brains reward center is triggered, which elevates the feel-good chemicals dopamine, oxytocin, and endogenous opioids. This gives you a great feeling, which is similar to whats known as runners high, and all that oxytocin is good for your heart.Being considerate of others will take you further in life than any college or professional degree. True Progress in False Hearts True Progress in False Hearts Hearts will never be practical until they can be made unbreakable. - The Wizard of Oz Yes, but I still want one. - The Tin Woodsman Our hearts make us human. But anatomically speaking they're just machines. Broken hearts of the emotional variety are perplexing. But fixing damaged organic hearts makes perfect mechanical sense to engineers. We can rebuild them; we have the technology. But do we have the right technology to reach the Holy Grail: a total, permanent, and fully implanted artificial heart? And can we replace that heart with something small, self-contained, and reliable enough to let patients live a relatively normal life? Maybe, says pioneering heart surgeon and researcher Dr. Albert Starr. Based on some of the exciting and radically divergent new designs about to hit the market, Starr believes science may soon declare victory in the 50-plus year race. Images: OHSU Heart of an Idea Starr's co-invention of the first successful prosthetic human heart valve changed the world of cardiac surgery, presaged the invention of the first mechanized hearts, and created a boom in cardiac device innovation that would directly advance the race to that ultimate goal. In 1958, Starr was a newly minted heart surgeon at the University of Oregon School of Medicine (now Oregon Health Science University, Portland) when retired hydraulics engineer Lowell Edwards approached him with a crazy idea. Edwards was determined to build a total artificial heart (TAH) and had a small fortune to invest, but he needed a partner fluent in physiology, anatomy, and surgery. As the only heart surgeon in town at the time, Starr was his man. Three Starr-Edwards heart valve prototypes. Image: OHSU Despite Edwards' urgency, the two agreed to start small. Starr knew any TAH device would require functioning mechanical valves which didn't exist yet. He also knew a prosthetic valve could save thousands of lives it its own right, even if the TAH project went nowhere. With that milestone as their initial goal, the two spent the next two years refining an elegantly simple caged-ball gadget that could support human blood flow for a lifetime. Marketed by Edwards' start-up company (now Edwards Lifesciences, Irvine, CA) the Starr-Edwards valve stood for decades as the gold standard of this new niche in the medical device industry. Starr and Edwards proved that an artificial heart was not only practical but a potential gold mine. The race was on. The search hit a big milestone in 1969 with the first human implant at the Texas Heart Institute (Houston). By 1982, the famous Jarvik 7 TAH was a household word after patient volunteer Barney Clark lived with one for 112 days. This model was used widely during the 1980s and is the predecessor of one of only two FDA-approved artificial hearts now on the market. Now sold as the SynCardia Systems (Tucson, AZ) temporary Total Artificial Heart, it was formally approved in 2004 for use as a bridge to transplant (BTT). In recent years, industry focus has shifted away from the TAH idea and toward more easily managed partial-assist devices such as BTTs. But the race isn't over. The number of available donor hearts annually serves only a fraction of the world's 100,000 Class IV heart failure patients, and the demand is growing. The need for a TAH as a destination therapy is growing with it. One promising solution in the wings is the French-designed Carmat the brain child of legendary surgeon Alain Carpentier of Paris's Pierre and Marie Curie University. Although technical details are still closely held, Starr believes his long-time friend and co-Lasker Award winner is working on a battery-powered, two-pump design incorporating new biopolymers and an advanced electronics system that synchronizes pumping action with a patient's exertion level. Be Still, My (Non)Beating Heart Thus far, all successful TAHs have been designed on nature's paradigm. They duplicate a patient's natural heart rhythmic beating to propel blood through the body. But is nature's way always the best way when it comes to automation? Starr says modern left ventricle assist devices (LVADs) that incorporate continuous-flow pumping systems could point to a new development: the no-pulse heart. That's the approach making headlines today out of the Texas Heart Institute, Houston, as surgeons O.H. Bud Frazier, M.D., and William E. Billy Cohn, M.D., advance their modified double-LVAD approach for total heart replacement. The team's ideas are largely based on the vision of surgeon-inventor-entrepreneur Richard Wampler, M.D. In the early 1980s, Wampler first championed the idea of a heart pump modeled not after nature but rather one of the oldest ideas in engineering: the Archimedean screw. He used the principle successfully in a tiny continuous flow device called the Hemopump. The size of a pencil eraser, the axial-flow pump was installed via a catheter into the aortic valve, where a screw element rotating at up to 25,000 rpm to transfer blood from the left ventricle to the aorta. Tests revealed no red blood cell damage, which is always a major challenge in cardiovascular engineering. Just as importantly, they revealed that humans are perfectly capable of living actively and safely with no trace of a heartbeat or pulse (provided they wear a medical alert bracelet in case of emergency). Wampler convinced Bud Frazier that axial-flow pumps had the potential to last longer, pump better, and fit into more patients than other designs. Their work began on a new generation of miniature implantable LVADs that evolved into the industry-leading HeartMate II (Thoratec, Pleasanton, CA). Frazier and Cohn are now working to adapt two state-of-the-art LVADs used in tandem for total heart replacement. After implanting their no-pulse heart successfully in dozens of calves, it was successfully tested in a human patient in 2011. Their 55-year-old volunteer survived for five weeks, but ultimately died from other complications of his advanced disease. Frazier's concept had been proven, and that's been enough to sustain a wider research and development effort, including an alternative double LVAD system under investigation at the Cleveland Clinic. Starr, still highly active in the field as co-director of OHSU's new Knight Cardiovascular Institute, marvels at this totally new direction. Lowell Edwards and I envisioned the prosthetic heart valve as a requirement for an artificial heart that pumped like the real thing, Starr says of his life-saving invention. Though you may instantly have your guard up around those who seem to always want to be the best-of-the-best, brace yourself: You need them. In fact, every company needs this type of personality.Why competitive people make us betterAs Los Angeles-based psychologist, Yvonne Thomas, Ph.D., explains, because competitive individuals are proactive, productive and creative they evoke energy into the working environment. âCompetitive people get things done and have much self-discipline, pe rseverance, and stamina, typically not giving up easily in the pursuit to be the best at whatever they are aiming for. Because competitive people are frequently very motivated and perform at a high level, they can often inspire others to function and perform to the best of their abilities as well,â she explains.The pitfall, of course, is how they handle themselves in both professional and personal settings. Because theyâre so laser-focused, they can forget their aggressive approach can be intimidating and at times, anxiety-inducing for others. Faced with competitive people, even some smart and motivated workers may decide that the best way to thrive is to not compete at all, rather than to compete and risk losing. So in the winter, with low levels of light exposure throughout the day, the timing of melatonin onset may be earlier, as the sleep systems in the brain become confused, leading to feelings of tiredness and lethargy.Tips:Try to spend more time outside in the winter months in order to get an adequate level of light exposure. Going for a walk in your lunch break is a good way to achieve this or walking/cycling to work or part of the way to work.Make sure your work and home environments are as light as possible during the day. Open your curtains and blinds as soon as you wake up in the morning and try to sit by a window at your place of work.In addition, dim your lights at home in the evening to encourage melatonin onset before you head off to bed.Physical and mental healthMost people are aware that we are more susceptible to colds and flu in the winter. This is not because cold temperatures cause these infections but because these viruses are able to survive better in colder environments. Another factor is that people spend longer indoors so are more likely to breathe in the same air as someone who already has an infection. In addition, the reduced exposure to sunlight leads to low levels of vitamin D and this can impair our immune system. The increased use of artificial heating in the winter produces hot, dry air and this dries out your mucus membranes, making your body more susceptible to infections such as colds or flu.Common symptoms of colds and flu are a blocked nose, sore throat, breathing problems, headaches, aching limbs and fever. Although infections can cause tiredness and an increased need for sleep, all these symptoms can disrupt your sleep making it harder to nod off or stay asleep. Adequate sleep is vital for effective immune function so these sleep disturbances can make you more vulnerable to colds and flu.Reduced levels of sunlight and vitamin D have been linked to low mood, irritability, fatigue and Seasonal Affective Disorder (SAD).This is a mood disorder characterised by depressive episodes that occur in the autumn and winter. Besides a persistent low mood, other symptoms of SAD include a lack of energy and feeling sleepy during the day, sleeping for longer and finding it hard to get up in the morning. These symptoms can in turn disrupt an individualâs natural sleep rhythms so it is more difficult to sleep at night time. Many people notice a lack of energy in the winter and occasional low mood but for SAD sufferers these symptoms are more persistent and debilitating. Youâve got to feel professional, so get dressed. You canât do it in your PJs - it wonât work.âAlso important is dedicating space to your job search and optimizing that space for the task at hand.Hurwitz recommends making the area in which you conduct your job search as much like your old workplace as possible. âAtmospherics are very important,â he said. âIf you feel like youâre at home and not working, then youâre at home and y ouâre not working. Do whatever you would do in your office. If you had a radio on for background noise in your office, turn on a radio. Donât change your work habits.
